Many U.S. and Canadian dental schools that offer advanced standing programs for international dentists participate in the ADEA Centralized Application for Advanced Placement for International Dentists (CAAPID) service. CAAPID provides a convenient way for qualified candidates to locate and apply to dental programs.  In order to be considered for admission to the IU-IDP program, international dental graduates must submit a single online application and set of credentials to the CAAPID program and pay the applicable fees. The IU-IDP program also requires a separate application and supplemental materials from applicants.

2013 ADEA CAAPID Application

Each IU-IDP applicant is responsible for providing all of the IU-IDP application forms and all supporting documents to the Indiana University School of Dentistry Office of Admissions and Student Affairs.   Please note that receipt of official school certified transcripts directly from the associated university of foreign dental schools attended is a legal requirement. They must be on file for all admitted students in the IU Office of Admissions at Indianapolis. The same is true for original score reports that are used as a part of the IU-IDP admissions process.

2. APPLICATION FEE

A nonrefundable application fee of $60 is also required. The fee and a graduate application should be submitted online at www.iupui.edu/~gradoff/admissions/  (Under Graduate Programs choose “Online Application”, create a guest account, then select “Dental Professional”, select “Summer 2013” term, only answer the questions with an asterisk (*)).  DO NOT SELECT “IU PAYPLUS”. 

3. PERSONAL STATEMENT AND DENTAL EXPERIENCE

Please provide dental education experiences, recent dental related activities, and professional goals in a personal statement typewritten on the separate form provided in the application packet. The personal statement should be limited to the space available on the form provided. A Curriculum Vitae (C.V.) and/or Continuing Education documents may be submitted by the applicant with the formal application.

4. DENTAL DEGREE

A notarized copy of the foreign dental degree of the applicant must be submitted to the Office of Admissions at the Indiana University School of Dentistry. If in a language other than English the dental degree must be accompanied by a notarized translation from a qualified U.S. translator.

5. OFFICIAL COURSE-BY-COURSE DENTAL SCHOOL TRANSCRIPT EVALUATION

Only an original ECE course-by-course evaluation of the applicant's dental school transcript will be accepted. Please make sure the document submitted is "Course-by Course Evaluation" and is issued directly to the Office of Admissions at the Indiana University School of Dentistry from Educational Credential Evaluators. www.ece.org.  A minimum grade point average of 2.5 is required. 

6. OFFICIAL SCHOOL CERTIFIED TRANSCRIPT

A certified copy is required of official credentials from all post-secondary schools the candidate has attended (foreign and domestic).  Transcripts/Marksheets should be issued by the University, not the dental college, and include a record of all studies completed to date; grades or examination results received (including failing and passing grades); maximum and minimum grades obtainable; rank in class; degrees, diplomas, and certificates earned; and length of school year.  The applicant’s certified transcript must be sent directly from the university of the applicant's foreign dental school to the Office of Admissions at Indiana University School of Dentistry in a sealed envelope. A U.S. or Canadian notarized photocopy of the applicant's foreign dental school transcript submitted by the applicant is not an acceptable substitute. The official school certified transcripts and all application documents become the property of Indiana University School of Dentistry and are not returnable.

7. NATIONAL BOARD DENTAL EXAMINATION 

National Board Dental Examination Parts I and II are both required for admission.   Please have the score report sent to you (do not open the envelope) and include the UNOPENED score report with the supplemental application materials to the Office of Admissions at Indiana University School of Dentistry.  No photocopies or faxed copies will be accepted.  A status of PASS is required of all applications for Part I.  For more information, please visit the American Dental Association (ADA) https://www.ada.org

8. TEST OF ENGLISH AS A FOREIGN LANGUAGE (TOEFL)

All applicants must be proficient in the English language.  Only applicants whose native language is not English are required to take the TOEFL.  An original score report of the applicant's TOEFL examination in the IBT format must be submitted directly to the Office of Admissions at Indiana University School of Dentistry (institution code 1325, department code #38) from the Educational Testing Service (ETS). No photocopies or faxed copies will be accepted. A minimum score of 90 or above on the Internet-based Test (IBT) version of the exam is required of all applicants. Preference is given to applicants who meet the following criteria:  Writing: Greater than or = 22; Speaking: Greater than or = 22; Listening: Greater than or = 20; Reading: Greater than or = 19.  Scores submitted should be no more than 2 years old.  www.ets.org/toefl

9. THREE LETTERS OF RECOMMENDATION

Applicants must submit up to 3 official school certified or original letters of recommendation written in English, or translated by a qualified U.S. translator, if the letter(s) are written in a language other than English. The letters must be written within 12 months of the date of the submission of the application and submitted directly to the Office of Admissions at the Indiana University School of Dentistry. Letters may be written by the Dean, an Associate Dean, department chair, or faculty member at the applicant's former dental school. At least one of the letters should be written by a dental professional who has worked with the applicant within the last 12 months. Telephone and address contact information for these individuals should be provided on the letters.

10. PERSONAL PHOTOGRAPH

Applicants must submit 2 passport size photographs of themselves, signed and dated by the applicant on the back of each photograph.

11.  UNITED STATES RESIDENCY STATUS

Applicants who are U.S. citizens must provide a copy of one of the following:

• U.S. Birth Certificate, or
• Permanent Resident/Green Card (front and back), or
• Passport page showing asylum status

12.  NAME CHANGE

Any applicant name change since completion of training must be verified by providing a copy of any legal documents showing the name change.  For example, in the case of marriage, a copy of a marriage certificate must be provided.

Individuals selected for an interview will be notified by mail and telephone approximately 2 to 3 weeks in advance of their date. Interviews are conducted when all application materials have been submitted as outlined.  The interview consists of an orientation and tour of the dental school, a pre-interview questionnaire, personal interviews with faculty, and a survey of dental subject knowledge.

Following the interview phase selected candidates will be invited to participate in a four week skills testing phase.  This 4 week period is designed to individually assess each candidate’s dental knowledge and skills in order to optimize candidate placement in the curriculum.  The class of up to 12 entering students will be selected from those individuals who present for the skills testing phase.  A nonrefundable one-time fee of $8,000.00 is required of applicants invited for this phase of the IU-IDP admissions process. Acceptance into the program is contingent upon satisfactory completion of the skills testing phase for determination of placement.   Please note that the skills testing fee does not guarantee acceptance into the program, nor is it applied toward tuition or other fees. 

After the interview/skills testing phases of the application process, and upon receiving an official letter of acceptance, applicants selected for the IU-IDP class starting in July 2013 are required to respond to the acceptance letter by the deadline reply date. Accepted IU-IDP students must meet and maintain all educational, financial, and immigration commitments to allow for their admission, registration, and enrollment at the IU School of Dentistry.

Acceptance into the IU-IDP is not a guarantee of eligibility for financial aid. Financial aid is awarded on the basis of financial need and citizenship to eligible students - those that are U.S. citizens or eligible non-citizens. A Free Application for Financial Student Aid (FAFSA) may be downloaded electronically from the US Department of Education to determine eligibility at www.fafsa.ed.gov.
