The Application Deadline is February 1^st, 2013.

The following information must be submitted before your application will be considered complete:

• IUPUI application: If you are not presently an IU student. You may apply on line through the IUPUI Admissions website: http://www.enroll.iupui.edu/transferstudents.shtml (This is a separate application that is required in addition to the dental hygiene application.). Please note: submission of the on-line application requires payment of the application fee using a credit card.
• Personal Statement: Please explain why you chose to pursue a career in dental hygiene and how you have prepared for this field. Describe any accomplishments you achieved, obstacles you cleared, or challenges you overcame that impacted your preparation for the dental hygiene program. Limit your response to 500 words. You will submit your personal statement while completing this online application.
• Observation Documentation: verifying a total of 8 hours of observation of a practicing dental hygienist at 2 separate practices on their office letterhead indicating your name, date and time of observation and hygienists’ name. The IUSD Student Records and Admissions Office must receive this documentation by April 1^st, 2013.
• SPRING GRADES and OFFICIAL TRANSCRIPTS:
  Spring grades must be submitted to the IUSD Student Records and Admissions Office no later than May 20^th, 2013.

The IUSD Student Records and Admissions Office will accept unofficial 2013 Spring grade reports, faxes and/or email from your instructors indicating grades in the required courses, followed by official transcripts.

It is your responsibility to report any change in current address, permanent address, email address, or telephone number to the IUSD Student Records and Admissions Office.  You are encouraged to apply early allowing time to correct any problems with your application or course schedule that may affect your eligibility.

All applicants are required to complete the Dental Hygiene Applicant Orientation.

Information will be sent to applicant after receipt of Dental Hygiene application. Failure to complete orientation requirement will result in the applicant not being eligible for consideration to the program. Note: Do not confuse this orientation with Dental Day which is held in October.
