International Applicants
International Applicants: A US citizen, permanent resident (green card), refugee, political asylee whose secondary education was outside of the US. Or a non-US citizen whose secondary education was outside and/or in the US.
Application Process: All applicants must submit an online University application.
A non-refundable, only payable by American Express, Discover, MasterCard, or Visa, is required in order to process application.

Official transcripts, including work already completed and work currently in progress. The records should show a complete list of courses taken, grades received, and any diplomas or certificates earned.
If the original documents are not in English, a verified translation must be sent with your official transcripts in your native language.
Educational Credential Evaluators (ECE) Course-by-Course Evaluation Report. Send all transcripts to ECE, PO Box 514070, Milwaukee, WI 53203-3470. We will accept a copy of this report.
Official National Board Scores (National Board Part I is required for all periodontic applicants)
Official Graduate Records Examination (GRE) scores (only required for MS dental Materials and Preventive Dentistry applicants). Please provide test site with the IUPUI institution code (R1325) and Dental Sciences major code (0604).

Supplemental Questions - the supplemental questions form is also available on the online University application. You may either upload the form to the online application or send with supplemental materials.
Certificate of Disciplinary History Form
If English is not your native language, you must demonstrate proficiency in English before you are admitted. TOEFL scores of 550+ (paper version/PBT) or 213+ (computer based version/CBT) or a minimum score of 79+ internet based version/iBT is required. IUPUI's school code for the TOEFL is 1325. A grade of 6.0 or higher on the Cambridge International English Language Testing System (IELTS) is also acceptable.
A completed Financial Support Agreement Form from each sponsor and all required supporting documentation. Please refer to the estimated fee sheet which lists all required expenses for all dental programs.
Letters of evaluation: Two letters of evaluation are required for the MS and MSD programs. Evaluations should be requested from a non-academic person unless you have extensive work experience with that individual and/or you have been away from academic institutions for some time.
Current Curriculum Vitae (CV) - A CV is a type of resume, most often used in academia that provides a summary of one's personal accomplishments, goals, professional achievements, employment history and research completed and any research publications.
A personal statement may be upload to the online application or sent with supplemental materials.
The personal statement should be up to 500 words in length and can be uploaded to the online application. In your own words, descript experiences that have shaped your interest in our program. Outline your professional goals; immediate and long term. In your personal statement, it is imperative that you cite your sources, if you include any statements or quotes that are not your original thought.
