Applicants are required to complete the university online application. The 2013 application will be avalable in April 2012.
A non-refundable, only payable by American Express, Discover, MasterCard, or Visa, is required in order to process application.

Two letters of recommendation - Recommendations should be requested from instructors who are able to comment on your qualifications for graduate dental study. They should not be requested from a non-academic person unless you have extensive work experience with that individual and/or you have been away from academic institutions for some time.
Official Graduate Records Examination (GRE) scores (required for MS Dental Materials and Preventive Dentistry applicants only). Please provide testing site with the IUPUI institutional code (R1325) and Dental Sciences major code (0604).

Current Curriculum Vitae (CV) - A CV is a type of resume, most often used in academia that provides a summary of one's personal accomplishments, goals, professional achievements, and employment history.
A personal statement may be submitted with the university online application or mailed with supplemental materials.
