In discussing his past medical history, you discover that Mr. Helmke has had some "heart trouble" since you saw him last. He says that he was diagnosed with "high blood pressure" and "a failing heart." The diagnosis was made about two years ago following a bout of chest pain. His physician advised him then to lose weight, begin a walking program, and reduce his salt intake. He also prescribed high blood pressure medication for him, although Mr. Helmke discontinued its use following some undesirable side effects. You notice that he appears to have a great accumulation of fat around his waist, and you suspect that he has not lost any weight. You ask you staff to telephone Mr. Helmke's physician.
