Directions for Online IUPUI International Application
All international applicants and US and permanent resident applicants who
have completed their dental degree at an international dental school are
required to submit an IUPUI international application. You may submit your IUPUI
international application online. You will be required to pay the $60.00
IUPUI application fee ($50.00 for permanent residents) at the time you submit your IUPUI international
application. MasterCard or Visa credit
cards are accepted. Please go to the following IUPUI international link
and follow the directions.

Under academic level click on graduate/professional
Hit the submit key
Scroll back up to the top of the page and click on step 7 – proceed with
the online international application
Indicate you wish to apply for summer 2009
If you have applied to IUPUI within the past 24 months, you do
not need to apply again. You may update your previous application
electronically by following the steps above and clicking on the previous
application link in step 7.
If you do not want to apply online, you may download the international
application form and application fee forms using the links below. If you
choose to download and fill out the international application and application fee
forms, please return both forms to the address listed below. You may send
the $60 payment in the form of a check or money order in U.S. funds, drawn on a
U.S. bank made out to "IUPUI - International Affairs". You may also pay by
credit card. Please use a Visa or MasterCard if you choose this option and
write your credit card information on the application fee form.
You do not need to send a paper application
or application fee if
you applied on line.
Do not submit duplicate
applications using different methods. Duplicate application fee payments cannot
be refunded.

Official TOFEL scores: Institution: IUPUI (code 1325)
All orthodontic applicants are required to complete and send official GRE scores. Institution: IUPUI
(code 1325); Department/School: Dental Sciences (code 0604)
All periodontic applicants are required to complete and send Part I of the
National Board scores (Part II is strongly recommended)
Graduate Programs not participating in PASS must request two letters of recommendation
(Programs: Dental Materials, Operative Dentistry, Periodontics, Preventive
Dentistry)
