GRADUATE INFORMATION FOR INTERNATIONAL APPLICANTS APPLYING TO THE DENTAL MATERIALS, ENDODONTICS, OPERATIVE DENTISTRY, PERIODONTICS, AND PREVENTIVE DENTISTRY GRADUATE PROGRAMS

Thank you for your inquiry regarding advanced study at Indiana University School of Dentistry.  Please refer to the Supplemental Checklist to insure that all application materials are sent directly to:

Do NOT submit application material directly to individual departments since this will only delay completion of your application file.

Application Materials

  • IUSD Graduate Application
  • School Certification of Disciplinary History Form: Submit this form to the dean of your school or a school official who has access to your disciplinary history.   The completed form should be placed in a sealed envelope and returned to the Office of Graduate Education.
  • IUPUI International Application and application fee  All international and US and permanent resident applicants who have completed their dental degree at an international dental school are required to submit an IUPUI international application.  You may submit your IUPUI international application online or fill out a paper version.   When applying on line, applicants are required to pay the $60.00 ($50.00 for permanent residents) application fee by credit card.  MasterCard or VISA are accepted.
    • In order to submit the online IUPUI international application, applicants must provide information according to their academic level and academic credential.
      • once you are at the IUPUI international website, select international for U.S. status and select Graduate/Professional for academic level.   Hit the submit key.
      • Scroll back to the top of the page and select step 7.  Proceed with the online application.  If you have applied to IUPUI within the past 24 months, you do not need to apply again.  You may update your previous application electronically by following the steps listed above.  Click the update your previous application link at step 7.
    • Directions for the IUPUI international paper application:  If you do not want to apply on line, you may download a paper IUPUI international application and send the paper application and the $60.00 application fee ($50.00 for permanent residents) to the address listed above.  Attach the International Admissions Application Fee Payment Form to your check or money order in U.S. funds drawn on a U.S. bank made out to "IUPUI - International Affairs".  Be sure that the check also has your full name written on it.  You may also pay by credit card.  If you choose this option, you may use a Visa or Mastercard, and write your credit card information on the international admissions application fee payment form.  The application fee cannot be waived or deferred.
  • Use only one method to submit the international application; otherwise, you will be charged a second international application fee that cannot be refunded.

  • Financial Support Statement
  • Two letters of recommendation from individuals who are familiar with your professional background and potential for advanced study.  Deliver the letter of recommendation form to the recommender, along with a stamped envelope addressed to the IUSD Office of Graduate Education.
  • For each college or university attended, the original transcript and diploma, certificate or degree statement.
  • A certified English translation of each of the above education records, if your school does not routinely issue official documents in English.
  • Official Report of TOEFL score:   All applicants from countries where English is not the official language and applicants who have had their training in a non-English speaking area must supply results of the Test of English as a Foreign Language (TOEFL).   A minimum TOEFL score of 213 (computer based), 550 (paper version) or 79 (internet based) is required for the graduate dental programs.   • GRE scores:  Required for MS Dental Materials, Preventive Dentistry and combined MSD Perio/Ortho Applicants: Institution:  IUPUI (1325); Department:  Dental Sciences (0604).
  • Official National Board Scores:  Periodontics Applicants:  Part I of the National Board Dental Examinations is required and part II is highly recommended.  Please have official scores sent to address listed below.  Dental Materials, Endodontics, Operative Dentistry, Preventive Dentistry Applicants:  The National Board Dental Examinations is not required; however, it is highly recommended that applicants complete Part I of the NBDE.

Application Deadlines

November 1, 2008 Operative Dentistry, Preventive Dentistry, Dental Materials and the Combined Operative Dentistry/Preventive Dentistry Program

The Combined Orthodontic/Periodontic Program deadline is September 2, 2008.

You must go through PASS in order to apply to this combined program.  The GRE examination is also required.  The PASS application will be available on the PASS website May 15, 2008.  The GRE is also required.  In order to receive scores in a timely manner, applicants are encouraged to complete the GRE by July 1, 2008.  To schedule a testing appointment call 1 800 GRE CALL (1 888 473 2255) or visit the GRE website   You may request that your scores be sent to 4 graduate institutions at no additional charge on your test date.  If you do not select institutions on the test day, you must pay to have your scores sent at a later date.  Send requests to:  Educational Testing Services, PO Box 6004, Princeton NJ  08541-6004.  Institution:  IUPUI (1325); Department/School:  Dental Sciences (0604).

Selection and Notification of Candidates

Selections, based on submitted credentials, are on a competitive basis as determined by the Graduate Admissions Committee. You will be informed of your status once selections have been made.

Degree Candidacy

Please note that if you are granted admission to Indiana University, you may be admitted as a "special student" not working toward a degree. In that case, you may be required to spend one semester as a special student so that your proficiency in didactic and clinical course work can be determined. At the end of this one semester you will be considered for admission to a degree program, and, if accepted as a degree candidate, be given credit for the graduate courses in which you were enrolled the previous semester, providing your grades were "B" or above.

Starting Dates

The graduate programs at Indiana University begin the first week of July each year. We do not accept students spring semester (January).

Status of Application

Applicants will receive notification of receipt of their application. The Committee will not review incomplete files, therefore, it is up to the applicant to verify completion of his/her file. Once the applications have been reviewed, applicants will be notified of their status.
