APPLICATION INFORMATION FOR US APPLICANTS APPLYING TO
THE DENTAL MATERIALS, ENDODONTICS, OPERATIVE DENTISTRY, PERIODONTICS, AND PREVENTIVE DENTISTRY GRADUATE PROGRAMS
Thank you for your interest in advanced study at Indiana
University School of Dentistry. Please review the
Supplementary Checklist to insure that all application materials are sent
directly to:

Application Materials

Graduate Application Form

School Certification of
Disciplinary History Form: Submit this form to the Dean of your dental
school or a school official who has access to your disciplinary history.
The completed form should be placed in a sealed envelope and returned to the
address indicated on the form.
- Application fee of $50, check or money order
payable to IUPUI. (This is a mandatory, non-refundable fee that cannot be
applied to other fees.)
NOTE: If you have or are currently attending Indiana University School of
Dentistry you do not have to pay this fee. If you are reapplying and
have already paid this fee, you do not have to pay the application fee
again.
- Two letters of recommendation should be requested from
instructors who are able to comment on your qualifications for graduate dental
study. Deliver the letter
of recommendation form to the recommender, along with a stamped envelope
addressed to the IUSD Office of Graduate Education.
- Official transcripts: Dental and undergraduate
transcripts from all college/universities you have attended.
- Official National Board scores.
- GRE Scores (required for applicants applying to the MS
Dental
Materials, Preventive Dentistry and MSD Ortho/Perio Combined Programs)

- Class rank/overall GPA must accompany application either in a letter by
a school official or on the transcript.
- Minimum D.D.S./D.M.D. GPA must be 3.0 or higher on a 4.0 scale.

All applicants who apply to this combined program must go through
PASS. The PASS application will be available on the
PASS website
May 15, 2008.
The GRE General Test is required for this program. In order to receive
scores in a timely manner, applicants are encouraged to complete the GRE
by July 1, 2007. To scheduled a testing appointment call 1 800 GRE
CALL (1 800 473-2255) or visit the GRE
website You may request that your scores be sent to 4 graduate institutions at
no additional charge on your test date. If you do not select
institutions on your test date, you must pay to have our scores sent at

Selection of Candidates
Selections, based on submitted credentials and personal
interviews (when required by department) are on a competitive basis.
Starting DatesAll graduate programs at Indiana University begin the first week of July each year. We do not accept students spring semester (January).
Status of ApplicationApplicants will receive notification of receipt of their
application. The Committee will not review incomplete files, therefore, it is
up to the applicant to verify completion of his/her file. Once the
applications have been reviewed, applicants will be notified of their status.
Rules Determining Resident and Nonresident Student Status for Indiana
University Fee Purposes:With very few exceptions, if you enter Indiana
University School of Dentistry as a non-resident, you will remain a
non-resident (for fee-paying purposes) throughout your postgraduate
education. Attending Indiana University School of Dentistry for one year
does not entitle you to resident tuition if you are not from Indiana.
