When you
need biopsy specimen kits you may call 317/274-7668, or send an e-mail to ds-opath@iupui.edu.
to
request how many kits you would like or mark the bottom of the Oral
Pathology patient history sheet for packs of 6 or 12. Requests are
processed daily and we will send your kits out ASAP. Please allow one week
for delivery. If your need is urgent, please let us know and we will try
to send the kits to you UPS next day. We cannot use P.O. boxes to deliver
specimen kits.
When using our kits, our providers
may choose between an Indianapolis area courier service, United Parcel Service
or United States Mail.
To use NOW courier:
If you are in the Indianapolis
area, you may call 638-6066 and give account number 4794 for pick up and
delivery of specimens from your office to our lab. This is a one-way
service, and we will continue to fax and mail the completed reports to
you. Call prior to 11:00AM for pick up and delivery by 5:00PM the same
day.
To use U.S. Mail:
Our biopsy mailers are printed
with a postage paid U.S. Mail label, and may be dropped in any mailbox with no
additional postage needed.
To use UPS:
If it is convenient for you to use
UPS, provided inside each box is a UPS label (standard ground or 2 Day Air,
depending on your proximity to Oral Pathology Group). To send your biopsy
via UPS, peel the label off and place it over the U.S. Mail label, then affix
your UPS tracking number to your patient file. If your biopsy does not
reach us in a timely fashion, you may call our office with your tracking number,
and we will track the package for you, or you may track it yourself at <http://www.ups.com/tracking/tracking.html>.
