To apply to the Indiana University School of Dentistry Ph.D. program, you will
use the University Online Graduate Application. Please read through all
information below before completing the online application.
Directions for Applying Online
Step One: Create an account
- Click on the online graduate application link in step two
- Click on create account and read the information on this page
- Write down your assigned personal identification number (pin) and create a
password. You will want this information on file so you can reenter the
application. You can complete the application in several sessions, if
necessary, save and logout.
- Re-enter the application with pin and password. When the application is
complete click on submit. A credit card is necessary to pay the $50 ($60
for international applicants) application fee online. This fee is subject
to change.
Step Two: Complete the
Online Graduate Application
Under Education Objectives, please be sure to indicate the following academic
program and major:
Academic Program -
Select: Dentistry (IU Graduate School)
Major - Select: Dental Sciences PhD
Semester you plan to enter/enroll: August - Fall Semester 2010
Supplemental materials need to be sent directly to the
IUSD Office of Graduate Education. These supplemental items are listed
below.
US Applicants: A US citizen whose secondary education
was in the US.
International Applicants: A US citizen,
permanent resident (green card), refugee, political asylee whose secondary education was outside of the US
or a non-US citizen whose secondary education was in the US.
