General Information

Mission Statement: Our Mission is to offer quality lifelong learning educational opportunities for dentists and allied dental personnel.

Registration Procedures: Applications for registration will be processed in the order received and on a space available basis. We suggest early registration for limited attendance programs and hands-on workshops. To secure a place in a course a completed registration form and either a full or prearranged partial payment must be received for each participant .

* All MasterCard and Visa credit card charges received for registration will appear on your credit card statements as "IUPUI Bursar."

Registration Fees: Registration fees include tuition, course materials, refreshments and lunch when indicated. Each dentist and or first office representative pays full tuition. Additional office representatives may attend at a reduced fee. To assist the fully retired or disabled meet there licensure requirements, special programs and fees have been arranged . Please call for further information.

Refund Policy: 25% of the tuition fee will be assessed if cancellation occurs within 14 days of the program; 40% of the tuition fee will be assessed if cancellation occurs within 3 days of a lecture program and no refunds will be granted the day of the program.  All cancellation fees are calculated back from 5:00 p.m. the day prior to the  lecture program. The balance remaining may be transferred to another lecture program, credited to your credit card, or you may request a refund check. Requests for refunds must be made within 2 weeks of the program date and directed to Dr. Donald E. Arens, Director, Continuing Education.  This refund policy does not apply to limited attendance lecture programs or combination lecture hands-on workshops.  Unless otherwise stated, No refunds will be granted 21 days prior to any limited attendance program or combination lecture / hands-on workshop.

Tax Payer Relief Act of 1997: Beginning in the tax year 1998, the Taxpayer Relief Act allows for a Federal income tax credit for any taxpayer that pays "tuition or registration fee" to attend non-credit instruction to acquire or improve their job skills. If a registration fee is paid by an individual to Indiana University School of Dentistry, IUSD is required to report this information to the IRS. Information is reported by the taxpayer social security number.
